How do I get to the Collection Site Locator?

Go to https://www.labcorp.com/wps/portal/findalab/

What steps do I need to follow once I am on the page?

Scroll down to the section where you enter the zip code. Enter the zip code of the area requested for a urine collection. Choose a mileage range in the mileage box; click on “Occupational” so that a site that is able to perform a urine collection is chosen.

What do I do if there are not any locations in my area?

Call HR ProFile Customer Service at 800-969-4300. HR ProFile will research the availability of an authorized third party collection site, and give you that information. If there are no authorized third party sites in the requested area, HR ProFile can set up a site for you. The required information is: name, address, phone number, and contact at the requested site. HR ProFile will then provide our clients with a form to be provided to the potential third party collection site. Using this form, the potential third party site completes it and sends it to Labcorp to become "authorized".

This request form requires the potential third party collection facility to use Labcorp protocols and pricing guidelines.

If accepted by Labcorp, the site will become authorized and HR ProFile’s standard third party collection charges will apply.

NOTE: if an unauthorized third party collection site is used, the client is responsible for the collection charges at whatever rate the unauthorized collection facility rate may be.

What will the applicant/employee be required to have when arriving at the collection site?

A picture ID and a SS number, as well as a Labcorp Chain of Custody form.

When will I be notified of the results?

Usually within 24 to 48 hours, depending on whether or not an MRO (Medical Review Officer) is used, whether the sample is positive or negative, and whether or not the sample is being tested for prescription drugs.
